What are DOT physicals?

A DOT physician is an exam the Department of Transportation requires before you can operate a commercial vehicle. These health screenings are different from annual physicals, which aim to prevent disease and illness. A DOT physical ensures you’re physically and mentally healthy enough to drive a commercial vehicle around civilians.

Who needs DOT physicals?

Every commercial motor vehicle (CMV) driver must undergo a DOT physical. These exams are required under federal law in every state. You must have a DOT physical if you:

  • Operate a vehicle that can carry more than 15 people
  • Transport hazardous materials
  • Drive a vehicle with a gross combination weight of 10,000 pounds or more
  • Receive payment for operating a vehicle that can carry more than eight people 

If you pass a DOT physical, you receive a medical examiner’s certificate (MEC) that allows you to perform these duties. Failing to obtain a valid MEC puts you at risk of losing your commercial driver’s license (CDL).

How often should I schedule DOT physicals?

All CMVs must undergo a DOT physical at least once every two years. If you have an underlying medical condition, such as diabetes, hypertension (high blood pressure), or chronic pain, you may need more frequent visits. What do DOT physicals involve?

DOT physicals are similar to annual health screenings, but they typically include a few extra steps. The physical exam is a head-to-toe evaluation that assesses your eyes, ears, nose, and throat. Dr. Aguirre also listens to your heart and lungs and palpates (touches) your abdomen before testing your flexibility, strength, and range of motion.

Along with the physical screening, Dr. Aguirre:

  • Tests your vision and hearing
  • Orders urinalysis
  • Checks your vitals (blood pressure, temperature, and respiration rate)
  • Records your height and weight

She may also order additional lab testing, such as bloodwork, to investigate any abnormalities during your exam.
